Photo and Video Model Release

The CLIENT + Subject hereby grant to Clay Fitness LLC and its legal representatives the irrevocable and unrestricted right to take, use and publish photographs/videos of the CLIENT or in which the CLIENT may be included, for editorial, trade, advertising and any other purpose and in any manner and medium; to alter the same; and to copyright the same. The CLIENT hereby releases Clay Fitness LLC and its legal representatives from all claims and liability relating to said photographs.

I have read this release of liability and fully understand its terms. I understand that I have given up substantial rights by signing it and I sign it freely and voluntarily without any inducement.
